Supporting Independence with Cascade3d Connected Care in Surrey

Cascade3d Connected Care have been working with the Reablement Service at Surrey County Council as part of a county-wide project introducing smart sensor techhnology. Since launching in February 2025, the project has supported almost 200 residents across Surrey.

The proactive monitoring system uses discreet motion sensors to provide insight into residents’ daily routines and wellbeing between planned care visits. This additional data has helped Reablement Teams personalise care plans, adjust visit schedules and where appropriate, support earlier discharge from the service.

Residents’ family members have also been able to access movement data through the Cascade3d mobile app, providing reassurance and greater visibility during recovery at home.

The project has been a great success and demonstrated the positive impact Technology Enabled Care can have across Reablement services.
